Facilitated Enrollment - Navigator - Community Health Advocate
Are you or your family in need of health insurance?
Funded by the New York State Department of Health, the IPA / Navigator program provides one-on-one assistance for individuals and small businesses interested in shopping for and enrolling in health insurance through The New York State of Health - the Official Health Plan Marketplace. Navigators are now available to meet with individuals, families and small businesses at various locations throughout the community to provide education about the different types of health insurance plans offered through the New York State of Health marketplace and to assist with enrollment into public insurance programs and qualified health plans.

As part of the Affordable Care Act, the S2AY Rural Health Network has received funding to assist small businesses in finding affordable health insurance. The program also helps small business owners learn about the laws and regulations surrounding the Affordable Care Act, and the Healthplan Marketplace. SBAP is an initiative of Community Service Society of New York, which has allocated funding to allow S2AY Rural Health to provide these services in Steuben, Seneca, Schuyler, Wayne, Ontario, Yates, and Allegany counties.
The Small Business Assistance Program provides health reform information in group presentations, one-on-one consultations, as well as services in any language. To schedule an appointment for assistance in applying for health insurance in the New York State of Health benefit exchange, Child Health Plus A & B; Family Health Plus and adult Medicaid call Helpline:
Through funding from NYS Department of Health, the S2AY Network offers enrollment assistance to individuals, parents applying for health insurance through New York State’s Child Health Plus Program and to parents and childless adults applying to New York State’s Medicaid/Family Health Plus Program. Residents of Schuyler, Steuben, Allegany, Yates, Ontario, Seneca and Wayne Counties meet with enrollment facilitators in convenient locations in their community to access no-cost information and to apply for coverage.
The Network has assisted over 19,000 uninsured children receive healthcare coverage since the program's inception.
